The Health Information Management Department is responsible for the issuance of the following health documents:
Requested documents through online
- Discharge Summary
- Laboratory Imaging & Histopath results
- Medical Abstract
- Medical Certificate
- Medico-Legal Certificate
- OB History form
- OR Records/OR Technique
- Release of Health Documents Monday to Friday excluding holidays (8:00 am to 4:00 nn)
- Request of Health Documents through online Monday to Sunday including holidays (anytime)
Note: Please bring the necessary SSS forms and other documents upon request of notification claims/insurances on HIMD window.
